The Russian system of support for medical decision-making Galenos is being tested at the Institute of Oncology named after N. N. Petrova.

The employees of the institute carry out an expert assessment of the applicability of the solution to systematize the treatment process and improve the quality of medical care in medical organizations in terms of oncology. For the project, the most common form was chosen - breast cancer. The successful implementation of this project will allow developers to expand the use of the Galenos system for another nosology.

The developer of the Galenos system is TechLAB LLC. Work on its development has been underway since 2014. The know-how of TechLAB is a technology that allows you to quickly digitize, structure and transfer clinical guidelines and other recommendation documents on various nosologies to the Galenos knowledge base. The Galenos system contains data from reference books, scientific articles, methods, treatment regimens and classifiers. This allows you to form treatment and follow-up plans in a few mouse clicks, maintain nosological and preferential patient registers, and analyze medical data. Galenos provides an opportunity to navigate the variety of existing treatment methods and make a doctor's decision, taking into account the characteristics of the disease of a particular patient.

Galenos can reduce the decision time or warn against mistakes, the system will tell the doctor what needs to be considered and what decision options may be. In this case, the SPPVR takes into account the choice of a doctor and this data in an impersonal form can already be further used to train the system and similar cases or scientific work. The EHR of the medical system of the institution with which it is integrated can be used as a source of data for the SPPVR. At the same time, the medical system gets the opportunity to use updated specialized reference books and classifiers of nosologies."

One of the key functions of the system is the ability to route patients. The Galenos system consolidates historical data to form individual patient cases from multiple healthcare organizations. This is especially true for those cases where patients can move from district cancer rooms to medical institutions at the regional and federal levels, the TekhLAB company notes.

The reason for choosing breast cancer, according to the project participants, was that it is the most common oncological disease in women in Russia: According to the Moscow Research Institute of Oncology. P. A. Herzen, breast cancer accounts for 21% of all cases, and 16.3% of all women die from this disease. In the world, the situation is similar: 25.2% of all cancer patients suffer from breast cancer, and among women, the incidence of this type of cancer is the highest - 43.3 per 100,000, according to the International Agency for Research on Cancer. The participants plan to expand the project by five more oncological nosologies.

Director of the Federal State Budgetary Institution “National Medical Research Center of Oncology named after N. N. Petrov "of the Ministry of Health of Russia, Professor Aleksey Belyaev agrees that" modern information technologies make it possible to provide the oncologist with new effective tools. The medical decision support system is one such technology that is actively used in world practice. The use of such systems even at the stage of diagnosis of malignant neoplasms could speed up the correct diagnosis, reduce the time it takes to send patients from regional institutions for treatment to the institute, systematize the collection of patient data, which generally contributes to improving the quality of medical care."

Earlier, Galenos was tested in hospitals in St. Petersburg and Arkhangelsk. The FSUE Research Institute of Hematology and Transfusiology, FMBA MH, SPPGMU named after I. Pavlova (31st City Polyclinic), SPbGBUZ City Hospital No. 15, SPbGBUZ City Clinical Hospital No. 31. The Galenos system has successfully solved the problem of improving the quality of information in the city register 7VZN and automating the formation of applications for preferential drug provision.

The use of intelligent medical systems, including medical decision-making assistance systems, helps to reduce the influence of the human factor on the process of making diagnoses and choosing the most effective way to treat the disease. The introduction of future medicine technologies will minimize the likelihood of medical errors, which are sometimes too expensive for patients.

The Government of the Russian Federation has declared its interest in the development of such medical innovations. In November 2017, by order of the Ministry of Health of Russia, a working group was created on the implementation of information systems to support medical decision-making, which also included specialists of TechLAB LLC, and in December 2017 it became known that the Ministry of Health of Russia intends to expand the federal project Electronic Health »By developing a support system for making medical decisions using elements of artificial intelligence. This was announced on December 13 by Prime Minister Dmitry Medvedev at a meeting of the Presidium of the Council under the President of the Russian Federation for Strategic Development and Priority Projects.
